New Health Forms for 2020

The new Annual Health and Medical Record (AHMR) form is now available. The changes you will see are minor, incorporating lessons learned from high adventure bases and jamborees. In addition, the Boy Scouts of America has compiled and reviewed comments from camp health officers, council health supervisors, and the medical community. Policies on the use of the AHMR have not changed.

Please discard any previous versions from your unit's files.  New copies will also be available at the Rolley Scout Service Center and you can also get to the new forms on this website under "Resources/Forms" on the left side of the home page. 

Although most questions are addressed on the updated AHMR FAQ page that is also available at the above link, it is important to note that 2020 will be a transition year.  If you have recently completed a Annual Health and Medical Record, that form is still accepted at all BSA activities for one full year from the time your physical was conducted.  You will not need to re-take your physical until the form expires.  However, make sure you have the current form when you go to your next physical.
